Breach Notification - Hospice of San Joaquin

On 7/2/2019 at about 12:50pm Hospice of San Joaquin experienced a cyber ransomware attack on its network. The data accessed may have included personal information such as full name, patient ID number, diagnoses, home address and other sensitive information. Though the malicious software accessed our servers, we do not believe, or have any indication patient or staff information has been utilized, disseminated or disclosed to unauthorized parties. It is pertinent to note all vendor and donor information is housed off of our servers and was entirely unaffected. 

 

Hospice of San Joaquin values your privacy and deeply regrets that this incident occurred. Hospice of San Joaquin is conducting a thorough review of the potentially affected information, and will report here and via our Toll Free Number [(888) 910-6365]if there are any significant developments. Hospice of San Joaquin has implemented additional security measures designed to prevent a recurrence of such an attack, and to protect the privacy of Hospice of San Joaquin’s valued customers.
