- For those who are terminally ill, a comprehensive plan of care: professional medical, social, spiritual and emotional support for the patient, family and caregivers designed together with each individual to fit her/his needs and choices.

- Practical, day-to-day support and guidance for the family/caregivers as they learn to adapt to changes in the health of their loved one.
- Hospice of San Joaquin serves our terminally ill patients in a variety of settings:
- Assisted living residences
- Skilled nursing homes
- Alzheimer–dementia facilities
- Independent living residences
- HSJ Hospice House

SERVICES
Hospice of San Joaquin provides:
- Visits, as the patient and family require or request, by the hospice nurse, social worker, chaplain, home health aide and volunteer
- On-call availability, 24-hours a day, of a hospice nurse
- Medical supplies and equipment
- Instruction in care giving and assistance with hygiene
- Respite care, support and practical help
- Counseling and spiritual support for the patient and loved ones, as they wish
- Longer-term bereavement support for surviving family members and loved ones, including children.

ELIGIBILITY
The staff and volunteers at Hospice of San Joaquin serve:
- Adults and children facing any life-shortening illness who, by referral from their doctor, are likely in their last year of life.
- Terminally ill people without regard to age, sex, race, religion, cultural or social differences or ability to pay.
- Patients and families who understand and choose the purpose of hospice care: quality of life, living as fully as possible, with focus on comfort rather than cure.
- Those in the community who have experienced the death of someone close to them are welcomed to the Bereavement Program. Adults, teens and children learn more about the grieving process and, when participating in structured groups, gain support and insight from others who have had similar experiences.
- All cities, towns and rural areas of San Joaquin County are served as well as portions of the counties of Solano (Rio Vista) and Sacramento (Isleton, Galt).

COST
The patient/family is not billed by Hospice of San Joaquin. The cost for the services of the Hospice of San Joaquin are covered by:
- MediCare, MediCal and some private insurance companies reimburse for core hospice services, i.e. nursing care, medication and some medical equipment.
- The generous support of the community through ongoing donations allows Hospice of San Joaquin to work with each patient and family regardless of ability to pay or insurance coverage. The patient/family is not billed by Hospice.
- If a patient is referred to the Hospice House, there is a separate room and board fee; however this may also be adjusted to the patient and family’s situation.
