When hospice patients are no longer able to remain in their own homes, Hospice House offers an alternative to home-based hospice care.

What is Hospice House?
Hospice House is the first and only residential care facility for hospice patients in San Joaquin County.   It is the sole development of Hospice of San Joaquin, a community based, not-for profit agency that has provided hospice services for terminally ill patients and their families in their homes throughout  San Joaquin County since 1982.

Why a Hospice House?
When hospice patients are no longer able to remain in their own homes, Hospice House offers an alternative to home-based hospice care.

Who may refer a patient?
Anyone may refer a patient to Hospice of San Joaquin; however, most referrals will come from health care professionals involved in the patient’s care.

Who qualifies to be a resident?
The Hospice House accepts patients who qualify and are enrolled in the services of Hospice of San Joaquin.  Hospice patients have been diagnosed with a terminal illness, have exhausted curative treatment and have a prognosis of approximately six months.  Certification for hospice may be renewed as necessary.  Hospice House patients are usually in their last six to eight weeks of life.

Admission to Hospice House is based on availability of rooms and prioritized by corresponding patient need. Admission criteria is utilized to review each case individually and objectively.

What are some reasons for Admission to Hospice House?

  • Alternative for patient with elderly or frail caregiver
  • Patient lives alone with no primary caregiver
  • Provide short respite stay to relieve family
  • Extra support in the final weeks of life or as symptoms  become too complex to manage well in the family home.

What is the cost of Hospice House room and board?
Medicare, Medi-Cal and some private insurance companies reimburse for hospice services. However, there is a nominal charge for room and board, at the Hospice House. A sliding scale based on patient's ability to pay is used.
