Hospice of San Joaquin is dedicated to serving terminally ill patients in San Joaquin County and the surrounding areas. We are pleased to join you and your staff in the continuum of care for your patients.

Our agency is a community-based organization that has provided nursing and supportive care to terminally ill patients in our community since 1980. We are Medicare and Medi-cal certified and JCAHO accredited.

  • Pain & symptom control
  • 24 hour on call nursing
  • Home Health Aide
  • Social services & volunteers
  • Music, pet & massage therapy
  • Bereavement follow-up for one year
    for the caregivers
  • For You and Your Staff

  • Primary Care management of your patients
    through phone calls and faxes
  • Interdisciplinary team summary updates every twenty-one days
  • Phone or in office consultation by our Education Coordinator
  • In-services to medical staff; CEUs
    offered to licensed staff

    For those not yet ‘registered’ with Hospice of San Joaquin, in case of medical emergency, please call 911 and the patient’s physician.
