Facing advanced chronic and terminal illness is difficult—for all involved.  Your loved one’s pain and symptoms, along with possible side effects of medication, can trigger feelings of isolation, hopelessness, and sadness.  Quality of life, as well as dignity and respect can become threatened.  The mission of Hospice of San Joaquin is to provide comprehensive and compassionate medical care for the patient and family as a unit, addressing social, psychological, and spiritual issues, as well as pain.  In fact, only when pain and distressing symptoms are managed, can there be any quality of life.

Hospice of San Joaquin provides services available to caregivers as follows:

Hospice Care — The philosophy of hospice allows the patient to remain at home, with family and familiar surroundings.  Through practical teaching and support, caregivers can feel confident in providing care to their loved ones.  This support allows the family and the patient to regain a sense of control over everyday choices and decisions.

Hospice House —Hospice House offers an alternative to home-based hospice care, by providing private rooms with 24-hour comfort care.  This option is available for those patients in whom pain and symptoms need closer monitoring, or who are close to the end of life.  There is a nominal room and board fee for Hospice House.

Transitions — This unique program is education-based, emotional support for patients and families seeking continued therapies.  We acknowledge coping with a life-limiting or advancing chronic illness can be challenging for both patients and caregivers.  Daily routines may be changing, care giving responsibilities taxing and medical information confusing.  So many changes are taking place, numerous medical choices to consider, and practical assistance arrangements to be made.  Where can you turn for information and support?  Transitions cares and is there to help…. when you need it most
