Transitions is a community service program whose main goal is to help individuals and their families cope with the problems that accompany life limiting or advancing chronic illness.    

 

     “When you or someone you love has advancing chronic or life limiting illness, life is affected in countless ways.  Daily routines often change, care giving responsibilities can be tiring, and medical information confusing.  So many changes taking place, numerous medical choices to consider, and practical assistance arrangements to be made.  Transitions cares and is there to provide information and support when you need it most”. 

What is Transitions?

Transitions is a community service program whose main goal is to help individuals and families cope with the problems that accompany life limiting or advancing chronic illness.  It is designed to improve participant and families’ quality of life through palliative education, emotional support, encouragement and community resource information as needed.  Education and support is provided via telephone communication or occasional visits to individuals in their homes or facility.  No hands on medical care is provided

Expert staff can help participants and their families by providing:

  • Palliative education about the disease process throughout all phases of illness or decline

       

  • Symptom relief solutions which facilitates comfort, control and choice     

  • Assistance with the development of a realistic health plan and care options             

  • Emotional support to decrease stress, minimize fears or feelings of aloneness

  • Community resource information to assist with financial, legal or practical home care concerns as needed

What is Palliative Care?

Palliative care affirms life, seeks to relieve suffering and enhance comfort.  According to the World Health Organization, palliative care is the focus on prevention, assessment and treatment of pain or other symptoms, and the provision of emotional and spiritual support. 

Who Qualifies for Transitions

Residents of San Joaquin County or neighboring communities

Individuals diagnosed with advancing chronic or life limiting illness

Individuals who are pursuing curative or palliative treatment

Individuals who are not ready for hospice care

Individuals who are at least 18 years of age

 

What is the cost of Transitions?

Transitions is a program sponsored by Hospice of San Joaquin, funded through grants, gifts and donations and is free of charge to participants.

 

How do I enroll in Transitions?

A referral to the Transitions Program may be initiated by individuals, care givers, medical personnel or facilities.  Physician and participant approval is required.  You may find the referral forms on the right top column under "How do I enroll into Transitions?"

Individuals have no obligation to enroll in Hospice at any time during their participation in the program.
