When hospice patients are no longer able to remain in their own residence, Hospice House offers an alternative to home-based hospice care.

What is Hospice House?

Hospice House is the first and only residential care facility for hospice patients in San Joaquin County.   It is the sole development of Hospice of San Joaquin, a community based, not-for profit agency that has provided hospice services for terminally ill patients and their families throughout San Joaquin County since 1982.

Why reside in Hospice House?

When hospice patients are no longer able to remain in their own homes, Hospice House may offer an alternative to home-based hospice care.  Some reasons to consider Hospice House are: 

 

  • Alternative for patients with elderly or frail caregivers;
  • Patient lives alone with no primary caregiver;
  • Patient may need extra support in advancing illnesses in the final weeks of life;
  • Symptoms/pain becomes too complex to be well managed in the home setting.

 

 

Who qualifies to be a resident?

Hospice House accepts patients who are currently enrolled in the services of Hospice of San Joaquin, or those with pain and symptom issues needing closer monitoring.  (Please refer to hospice care tab if you need details on hospice services eligibility.)  In addition, patients admitted to Hospice House should:  

 

    • Have a life expectancy of two months or less (based on their physician’s prognosis);
    • Be at least 18 years of age.

Admission to Hospice House is based on availability of rooms and prioritized by corresponding patient need. Admission criteria are utilized to review each case individually and objectively.

What is the cost of Hospice House room and board?

Hospice House has a nominal fee for room and board. The charge is determined on a sliding scale based on the patient's ability to pay.  Please note:  Hospice House room and board is not covered under the hospice benefit, or any other insurance, and is the sole responsibility of the patient/family.
